The metal pad (the shiny part) on top of the actual pedal mechanism can come loose, slide upwards and get caught in the fabric trim. It doesn't look like there's much holding it in place, except possibly glue.

The process need to be designed in such a way that the "people" performing it literally cannot mess it up, or have checks in place to verify it is done right. Passing the blame to your bottom of thr barrel employees is no excuse since you're responsible for hiring them.

According to Sandy Munro, there's a mantra when designing how the vehicle is to be assembled. It was something similar to "One eye closed and one arm tied behind my back."— the message being a vehicle should be designed in such a way that assembling it is easy and leaves as little room for mistakes as possible. It's why cars no longer use philips screws and instead use torx screws and parts that snap or clip into place.
